Hands-On Healing: Neurodynamic Techniques for Carpal Tunnel Breakthrough!

Carpal tunnel syndrome (CTS) arises when the median nerve, located on the palm side of the wrist, is compressed. This condition is characterized by motor and sensory issues, including numbness, pain, and tingling in the first three fingers, along with muscle weakness and reduced hand function. For those with mild-to-moderate CTS, non-surgical treatments are recommended prior to considering surgery, with chiropractic care being a viable option. Chiropractors employ neurodynamic techniques among their treatment methods.

Neurodynamic techniques involve manual therapies aimed at improving the length and mobility of nerves. For CTS, these techniques target the median nerve not only at the wrist but also along its path from the neck down to the shoulder, elbow, and forearm. Addressing restrictions of the median nerve upstream from the wrist is crucial, as these can mimic CTS symptoms and heighten the likelihood of nerve entrapment at the wrist. Without assessing the entire path of the median nerve, treatment outcomes may be less effective.

A systematic review and meta-analysis in July 2023, which examined twelve randomized controlled trials with 1,000 mild-to-moderate CTS patients, demonstrated the efficacy of neurodynamic techniques in alleviating symptoms and enhancing wrist and hand functionality. This review also confirmed improvements in median nerve function, as indicated by nerve conduction tests. Similar positive outcomes were reported in a 2022 systematic review of six randomized controlled trials involving 401 CTS patients, showing immediate enhancements in pain, functionality, and nerve conduction following neurodynamic treatments.

Highlighting the significance of a comprehensive approach over treating just the wrist, a 2022 study divided 62 CTS patients into two groups—one receiving treatment solely on the wrist and the other treated both at the wrist and along the median nerve's path. The group receiving broader treatment reported more significant improvements in pain, grip strength, and functional status.

Conservative chiropractic management of CTS may also encompass additional manual therapies, use of nocturnal splints, exercise regimens, dietary advice, and physiotherapy modalities such as electrical stimulation, ultrasound, laser, and pulsed magnetic field therapy. Chiropractors may collaborate with other healthcare professionals for treatments that extend beyond their expertise when necessary.
